- Identification of Removed Material: If applicable, identify the material that has been removed or disabled and the location where it appeared before removal or disabling.
- Statement of Mistake or Misidentification: A statement under penalty of perjury that you believe the material was removed or disabled due to a mistake or misidentification.
- Jurisdiction and Service Acceptance: Your name, address, and telephone number, along with a statement consenting to the jurisdiction of the federal district court for your judicial district or any judicial district where the service provider is located if you are outside the U.S. and that you will accept service of process from the complainant.
